Job Title: RN Case Manager Job Specialty: Case Management Job Duration: 13 Weeks Shift: Monday - Friday, 9:00 AM - 5:00 PM (No on-call or weekends) Guaranteed Hours: N/A Experience: Minimum 2 years of RN Case Management experience, with long-term acute care setting experience. License: Kentucky RN License Certifications: BLS certification (must be through the American Heart Association) Must-Have: Experience in discharge planning, clinical case management, and utilization review. Job Description: - Coordinate the management of care to ensure the optimal utilization of resources, service delivery, and compliance with external review agencies. - Provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and evaluation of individual patient needs. - Enhance the quality of patient management and satisfaction to promote continuity of care and cost-effectiveness. - Integrate functions of case management, utilization review, and discharge planning. - Serve as a patient advocate through resource utilization, discharge planning, and addressing holistic patient needs.
