Job Description:
Our Company:Abode Care PartnersOverview:In this full time,
clinical physician lead role, under supervision of the CMO, you
will have several clinical responsibilities, along with some
specified administrative responsibilities. While providing
day-to-day clinical care to residents residing in facilities as
well as home bound patients, you will practice and role-model
ethical and evidence-based decision-making to promote the vision
and mission of the company to other practitioners and
staff.Responsibilities:Provides high-quality complex and geriatric
care to residents and patients across settings in partnership with
non-physician practitioner and other interprofessional team members
to an identified panel of patients across various assisted living
and skilled nursing facilitiesKeeps high engagement with facility
leadership teams, nurse practitioners/physician assistants,
patients, and familiesProvides collaborating physician duties for
on average of 5 but up to 15 nurse practitioners and physician
assistants (based on state regulatory parameters) to support them
in their clinical rolesBased on state and regional regulatory
frameworks, audits up to 10% of practitioner medical orders and/ or
documentation for each provider monthlyIf needed, assumes medical
directorships for skilled and/or other senior living
facilitiesHelps identify new growth opportunities with skilled
facilities and/or other senior living facilities in the
market/regionParticipates in company frontline /backup call
processes for timely response to acute changes of condition to
prevent avoidable hospital transfers, or to meet other clinical
needsKeeps a high focus on quality outcomes including but not
limited to care path compliance and productivity, prevention of
avoidable transfers and decrease in polypharmacy and costs of
careAttends required trainings, and meetings as mandated by the CMO
or other leaders to assure best care delivery by the
companyQualifications:Education/Experience:Graduate from an
accredited school of medicine and is a Doctor of MedicineTwo or
more years of administrative leadership experience in
healthcareClinical medical experience of seven years or more
preferredCertifications/License/Other Special RequirementsBoard
certification in Family Medicine, Internal Medicine or Geriatrics;
qualification in Geriatrics preferredPracticing physician currently
licensed under the provisions of the State Board of MedicineActive
unrestricted licensure to practice in state of employmentCurrent
Drug Enforcement Administration registrationQualifications:Ability
to communicate effectively with patients and other healthcare
support staffAbility to treat patients and their families with
compassion and understandingAbility to work for long periods of
time with patients who need special attentionAbility to display
patience with children and adult patients who fear medical
treatmentExcellent computer and electronic record-keeping
skillsExcellent organizational and management skillsExcellent
decision-making, problem-solving and cognitive abilitiesAbout our
