POSITION TITLE: PrEP Prevention Navigator (PAPI program) REPORTS TO: Prevention Manager SUPERVISES: N/A **This position does offer a hybrid work schedule, where some work is done remotely from home, some in office, and some out in the community. We will be hiring 2 candidates.**This position does not require a social work license. POSITION SUMMARY: This position facilitates access to Pre-Exposure Prophylaxis (PrEP) and provides assertive, community-based interventions and case management for high-risk HIV- individuals. Offer linkage to PrEP providers, PrEP medications, health insurance coverage, drug and/or alcohol treatment, syringe exchange programs, and holistic care resources; provide advocacy and psychosocial interventions. Provide outreach services to at-risk persons including HIV testing, education/health literacy and intervention services. RESPONSIBILITIES: Provide outreach, HIV testing and health education for high-risk HIV- individuals who are candidates for PreExposure Prophylaxis (PrEP). Ensure access to HIV preventive medical care by helping clients to obtain and maintain health insurance, benefits/entitlement programs, and other sources of support for covering costs of medications and medical care. Assist clients in enrolling in PrEP access and insurance programs; enter data into billing systems. Maintain timely and accurate client service data and case management records as required by regulatory or funding agencies and Caracole; meet all required deadlines for documentation. Act as liaison between clients, insurers, state of Ohio and medical providers to assure PrEP access; follow up on payment and laboratory results to support medical adherence to PrEP. Provide basic case management services, including initial assessments, individualized service planning, and ongoing updates and contacts with HIV- individuals enrolled in PrEP access programs. Provide advocacy and education to area providers about HIV prevention, PrEP/PEP, PrEP assistance programs, and patient access. Maintain up to date knowledge about medical interventions to prevent HIV; act as a resource and support to staff, clients and client family members for PrEP, PEP and other prevention methods. Effectively utilize a broad and comprehensive knowledge of community resources serving those at high risk of infection, such as syringe exchange programs, use of Narcan, medical and mental health/substance abuse treatment providers, and social support services. Represent Caracole in the community and develop partnerships and strategies with service providers in Greater Cincinnati. Adhere to all rules, policies and regulations pertaining to privacy and confidentiality. Adhere to the harm reduction model as it relates to individual client care and public health. Adhere to Caracole standards of care. Participate in quality management activities; individual/group/team supervision meetings and educational/training events. Maintain a collaborative working relationship with Caracole team members. Identify and pursue educational opportunities to promote professional growth. Other duties assigned. QUALIFICATIONS:Associates' or Bachelor's degree in related field with at least six months experience in a social service setting, or equivalent experience. Basic knowledge of HIV/AIDS and bio-medical prevention interventions, substance abuse service networks, entitlement programs, medical and social community services. Proven ability to work harmoniously with ethnically, culturally, and racially diverse groups of individuals. REQUIRED SKILLS: Ability to maintain confidentiality in verbal, print, and electronic communications. Good computer skills, including competency with Microsoft Office and database entry. Read, write, speak and understand English with good verbal and written communication skills. Demonstrated sensitivity to persons living with chronic disease (HIV/AIDS, HCV and/or addiction) and co-existing social, mental and physical conditions. Must work well independently and be a strong member of a small team. Excellent attention to detail. Bilingual (Sp/En) preferred. PHYSICAL REQUIREMENTS/WORKING CONDITIONS: This hybrid, hourly position includes time spent working in office, remote at home, and in the community. Community work includes hospitals, jails, treatment centers, shelters and other key locations for access to high risk clients. Work is generally 9-5 M-F with occasional pre-scheduled weekend and evening hours. The employee is frequently required to talk, hear, operate a telephone, fax machine and computer with or without accommodation. The employee is required to move throughout the building and to other sites in the community. Employee must have valid driver's license, reliable transportation, and ability to travel to other sites and meetings as needed. SALARY:This position is full time, hourly: $21.63/hr (approx. $45,000 annually).
