- Certification Details
- BLS - Basic Life Support
- Nursys - Prefer Compact Nursing License
- State or Compact License
- NIH Certification - if required within X days
- Additional Details
- COVID-19 vaccine required, provide vaccination card with

- Influenza vaccine required. Review for religious or medical

- Physical capacity statement required stating the candidate is

- Standard 10-panel drug screen required.
